Why Are All My Tests Normal If I Still Feel Terrible?

If you've ever been told, "Good news, everything looks normal," while you still felt terrible, you're not alone. It's one of the most common stories I hear. For many people, the frustration isn't just the symptoms. It's wondering why they still feel this way when every test says everything looks fine.

The mechanic analogy

Imagine taking your car to a mechanic because the engine is sputtering and stalling. The mechanic opens the hood, checks for broken parts, and tells you: "Everything looks good."

You'd probably be confused. Your car clearly isn't running the way it should. The problem isn't necessarily that the mechanic is wrong. It's that they only looked for certain types of problems.

Some tests look for broken parts. Other tests evaluate how well the engine is actually performing. The human body works much the same way.

Many medical tests are excellent at identifying structural problems such as fractures, tumors, bleeding, infections, or major disease. But many persistent symptoms happen because something isn't functioning the way it should, even though the structure itself appears normal.

Structure vs. function

Structure asks whether something is physically there, damaged, or missing. Function asks how well a system is actually working. Both matter, but they answer different questions.

Structure often answers questions like: Is something torn? Is something broken? Is there bleeding? Is there a tumor?

Function asks different questions, such as: How well are the eyes working together? How well is the balance system processing information? How efficiently is the brain responding? How do symptoms change during movement?

An MRI might show that nothing is structurally wrong with your brain. That is genuinely good news. But it doesn't necessarily tell you how well your brain is coordinating balance, vision, movement, or sensory input in real time.

Why this matters

When testing comes back normal but symptoms continue, many people wonder if it's "just anxiety" or if they're imagining things. Persistent symptoms deserve to be understood.

Anxiety can play a role in how symptoms feel, but that doesn't mean the underlying problem is "all in your head" in the dismissive way people sometimes use that phrase. Understanding why symptoms occur is often much more helpful than simply giving them a label.

Patterns matter

Symptoms often follow patterns. Busy grocery stores, driving, darkness, turning the head, fatigue, and stress can each tell a different story.

The goal isn't to collect diagnoses. The goal is to understand the pattern. Paying attention to when symptoms appear and what changes them is often one of the most useful things you can do.

Why this can happen

  • Routine imaging and lab work are often designed to rule out structural disease, not to measure how sensory systems communicate under real-world demand.
  • Balance, vision, vestibular processing, and proprioception can create mismatch symptoms even when no structural lesion is found.
  • Compensation can reduce symptoms in some settings while leaving the system fragile in others, which is why people can look "fine" on paper and still feel terrible day to day.

What people often misunderstand

  • A normal MRI means nothing is wrong. It usually means nothing structurally dangerous was found. It does not automatically explain function.
  • If symptoms are real, anxiety must be the cause. Anxiety can amplify symptoms, but it is not always the primary explanation.
  • Feeling better after rest or avoidance means the problem is solved. Reduced symptoms are not always the same as restored function.

What I look at

  • How the vestibular, visual, and proprioceptive systems are contributing under different conditions
  • Eye movement and balance findings that can be measured and retested
  • Which environments, movements, or sensory demands provoke or ease symptoms
  • Whether the current strategy is compensation, adaptation, or genuine improvement
