Why do I feel like I'm walking on a boat?

Feeling like you're walking on a boat is one of the most common ways people describe chronic dizziness. Some people feel like they're rocking. Others describe floating, bobbing, swaying, or feeling as though the ground is moving beneath them. While these sensations can occur for many different reasons, one pattern I commonly see in my practice is dysfunction involving the otolith organs, which help your brain detect gravity and straight-line movement. The important question isn't simply what the sensation feels like. It's understanding why your brain is creating that sensation.

What causes the "walking on a boat" feeling?

Your vestibular system isn't just responsible for detecting spinning.

It also contains two structures called the otolith organs, which constantly tell your brain which way is up, whether you're moving in a straight line, and how your head is positioned relative to gravity.

If those signals aren't being processed efficiently, people may describe sensations such as:

Walking on a boat

Rocking

Floating

Bobbing

Swaying

Feeling like they're still moving after they've stopped

These sensations often feel very different from the classic spinning vertigo that many people associate with vestibular disorders.

What about Mal de Débarquement Syndrome?

People with these symptoms are sometimes diagnosed with Mal de Débarquement Syndrome (MdDS), which literally translates to "sickness of disembarkment."

The diagnosis describes the sensation of feeling as though you're still moving after getting off a boat, cruise ship, airplane, or other form of transportation.

In my practice, however, I don't stop at the diagnosis.

One pattern I commonly find is objective evidence of otolith dysfunction, even in people who have already been given an MdDS diagnosis.

That doesn't mean everyone with MdDS has the same underlying dysfunction.

It means I want to understand how your vestibular system is functioning, rather than assuming the diagnosis tells the whole story.

Why are the symptoms often worse with your eyes closed?

Many people tell me the rocking or floating sensation becomes much more noticeable when they close their eyes, especially while trying to fall asleep.

That makes sense.

When your eyes are open, your brain can use visual information to help stabilize your perception of movement.

Once your eyes are closed, your brain relies much more heavily on the vestibular system and other sensory inputs.

If those systems are not functioning efficiently, the sensation of rocking or movement may become much more noticeable.

For many people, this makes falling asleep one of the most frustrating parts of the day.

What I look at

  • When someone tells me they feel like they're walking on a boat, I don't immediately assume they have one specific diagnosis.
  • I want to understand why they feel that way.
  • Using objective testing, I evaluate different parts of the vestibular system, including the otolith organs, along with eye movements, balance, and other systems that help your brain understand movement and position.
  • The same sensation can occur for different reasons in different people.
  • That is why I focus on identifying the specific dysfunction that may be contributing to the symptoms, applying targeted rehabilitation, and objectively measuring whether those findings improve.
